Requirements

Tractors
• Tractor must be within 7 yrs of current year.
• Must be clean and undamaged in physical appearance and approved by PTI.
• Must have 3 axles.
• Sleeper required.
• Headache rack required.
• Ability to scale 48,000 lbs.
• Must be able to pass PTI DOT annual inspection.
Trailers (Local/Regional ONLY)
•Must be at least 48ft long
•Must be road worthy and pass annual federal inspection
•Must provide trailer registration/copy of title in driver’s name

Cargo Equipment
• Ten Chains and Binders (of at least 5/16” grade).

Qualifications
• Must be at least 23 years of age.
• One year OTR experience in the past three years.
o With at least 6 months verifiable flatbed experience.
• Steady work history. Excessive work history and preventable service failures may disqualify drivers.
• Must be able to provide physical street address
o Address provided must be reliable for regular receipt of company communications.
• Must have working telephone number.
• Felony convictions within the past seven (7) years are disqualifying.
• Failure to disclose a misdemeanor or felony conviction on the application can be disqualifying for falsification of the record.
• No Drug related convictions within the past seven (7) years.
• Driver’s found to be on criminal probation must be qualified by HR. Condition hire letters will be required.
• Must have at the time of hire, and maintain during employment, an acceptable DOT Driver Safety and Performance score with a rating of “Continue to Operate” as tracked by FMCSA within the 7 basic evaluation areas. For Example, marginal or unfit ratings within “unsafe driving” and “fatigue driving” areas may result in disqualification.
• Must own the vehicle. Exceptions to this must have approval from CFO and Operations Manager.
• Must have occupational accident insurance coverage of $1,000,000.00 or more. May provide own coverage, or may purchase through PTI.
• 1 million non-trucking liability bobtail insurance. May provide own coverage or purchase through PTI.
• Driver must provide own base plate or can request through PTI.
o If qualified driver may apply for base program offering free plates as long as driver has been with current employer 2 yrs or more and can produce no less than $12,000 gross each month for PTI.
• Must provide IRS date stamped schedule of Heavy Highway Vehicle form or otherwise referred to as 2290.
• Must have Federal ID Tax number (normally printed on 2290).
• Copy of the title in driver’s name.
• Copy of current cab card or a BOS (bill of sale) or MSO on new vehicle with full vehicle description, including full VIN # with date and authorized signature from seller.
• Must have Texas Worker Identification Card (TWIC) or have applied and provide registry code.
Unsafe Driving:
• No more than 4 moving violations on MVR within the past 3 years combined on MVR and/or PSP reports. Any exception to hire a driver that exceeds the minimum criteria must be approved by the Director of Human Resources and Safety Director.
• Driver who has a serious traffic violation within the last three (3) years must be approved by the Director of Human Resources and Safety Director.
o Speeding excessively 15 mph or more
o Careless and/or Reckless Driving
o Making improper or erratic traffic lane changes
o Following the vehicle ahead too closely
o Driving a CMV without proper endorsements
Crash and Accident History:
• No preventable accidents or more than 2 preventable incidents in the past 12 months, or 3 accidents in the last 3 years as reported on MVR, PSP report and/or employment verifications. Any exception to hire a driver that exceeds the minimum criteria must be approved by the Director of Human Resources and Safety Director.
Driver Fitness:
• Must have Valid CDL license from state in which the driver resides (or must be able to obtain within 30 days after hire).
• Suspension of CDL license within the last five (5) years must be reviewed by Safety prior to qualification.
• Must meet all applicable D.O.T regulations (including a DOT physical and pre-employment drug test and company’s driver performance test-DPT).
• Must be eligible to work in the U.S. and provide appropriate records of proof.
• Not more than 5 driver fitness related violations within the past year as indicated on MVR and/or PSP report. Any exception to hire a driver that exceeds the minimum criteria must be approved by the Director of Human Resources and Safety Director.
Driver Fatigue:
• Not more than 4 log or hours of service violations within the past year as indicated on the MVR and/or PSP reports. Any exception to hire a driver that exceeds the minimum criteria must be approved by the Director of Human Resources and Safety Director.
Controlled Substance and Alcohol:
• No D.U.I./D.W.I’s in the last 5 years and no more than one in a lifetime in any vehicle.
• No refusal to take Controlled Substance or Alcohol Test – Includes Pre-Employment.
• No drug related convictions EVER.
Vehicle Maintenance:
• Not more than 5 maintenance related violations within the past year as indicated on the MVR and/or PSP reports. Any exception to hire a driver that exceeds the minimum criteria must be approved by the Director of Human Resources and Safety Director.
Cargo Related Compliance:
• Not more than 3 cargo related service violations within the past year as indicated on the MVR and/or PSP reports. Any exception to hire a driver that exceeds the minimum criteria must be approved by the Director of Human Resources and Safety Director.
